Location: Santa Monica, CA

Schedule: Full-Time | 8-hour Day or Variable Shifts

Compensation: $109,116 – $172,744 annually

Incentives: $10,000 hiring bonus (for eligible external candidates)

About the Role

We are seeking an experienced Care Manager RN to join our team in Santa Monica, CA. This role is responsible for care coordination, utilization management, and supporting patients across the continuum of care.

Case Management is a collaborative practice involving patients, caregivers, nurses, social workers, physicians, payers, and community partners. The Care Manager plays a key role in facilitating communication, ensuring effective transitions of care, and recognizing the impact of social determinants of health on patient outcomes.

 

 

Key Responsibilities 

  • Coordinate and manage patient care plans across multiple settings

  • Collaborate with healthcare providers, caregivers, and payers to ensure safe and effective transitions of care

  • Support utilization management and resource allocation

  • Advocate for patients’ rights and self-determination while addressing complex care needs

  • Document assessments, plans, and outcomes in alignment with professional and organizational standards

 

 

 

Required Qualifications 

  • Bachelor’s degree in Nursing (BSN) or equivalent education/experience

    • RNs with 20+ years of nursing experience are not required to obtain a BSN

     

  • Master’s degree in Nursing or a healthcare-related field

  • Current California RN license

  • Minimum 2 years of experience in a healthcare-related setting (acute, ambulatory, post-acute, etc.)

  • Minimum 2 years of experience in Case Management (Care Coordination or Utilization Management)

    • OR completion of the Transitions in Practice (TIP) program for Care Managers (must have experience in the same type of nursing unit)

     

Preferred Qualifications

  • National certification in Case Management (CCM or ACM)

 

Compensation & Benefits

  • Base Salary: $109,116 – $172,744 annually

  • Hiring Bonus: $10,000 for eligible external candidates
