Job Description


We are seeking a collaborative Nurse Practitioner to join our innovative interdisciplinary team. located in California. In this role, you will have the opportunity to utilize your expertise to provide comprehensive, holistic care to seniors living independently in their homes and communities.




As a Nurse Practitioner, you will conduct assessments, develop care plans, and provide primary care services. You will work closely with physicians and other care team members to coordinate care and ensure positive outcomes for participants. This is a chance to make a difference in seniors' lives while working in an encouraging, supportive environment.




We are seeking candidates with:
  • Nurse Practitioner certification and prescriptive authority
  • Minimum 2 years experience as a NP, preferably caring for seniors
  • Passion for improving seniors' wellbeing and enabling them to age in place
  • Strong collaboration and communication skills
  • Valid driver's license and clean DMV record
