Job Description
As a Licensed Practical Nurse (LPN), you will play a vital role in providing direct patient care and support to healthcare teams within our Senior Living Facility. Under the supervision of registered nurses (RNs) and physicians, you will administer medication, assist with patient assessments, perform routine procedures, and provide compassionate care to patients of all ages. Your attention to detail, clinical skills, and dedication to patient well-being will contribute to the delivery of high-quality healthcare services.
Responsibilities:
- Provide direct patient care, including administering medications, performing wound care, and assisting with treatments and procedures as directed by the healthcare team.
- Conduct patient assessments, including vital signs, physical examinations, and medical history reviews, and report any changes or concerns to the supervising RN or physician.
- Assist patients with activities of daily living, such as bathing, dressing, feeding, and mobility, while promoting independence and maintaining dignity.
- Document patient care activities, observations, and interventions accurately and timely in electronic medical records (EMR) or other documentation systems.
- Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans based on patient needs and goals.
- Educate patients and their families about health conditions, treatment plans, medication instructions, and self-care techniques to promote wellness and prevent complications.
- Ensure compliance with regulatory standards, infection control protocols, and facility policies and procedures to maintain a safe and therapeutic environment for patients and staff.
- Participate in quality improvement initiatives, safety audits, and staff meetings to contribute to the delivery of high-quality, patient-centered care.
- Stay updated on current nursing practices, evidence-based guidelines, and professional development opportunities to enhance clinical knowledge and skills.
- Demonstrate professionalism, empathy, and cultural sensitivity in all interactions with patients, families, and colleagues to promote a positive healthcare experience.
Qualifications:
- Current license as a Licensed Practical Nurse (LPN) in the state of Pennsylvania.
- One year experience in a healthcare setting, preferably in long-term care, rehabilitation, or acute care.
- BLS/ACLS Certification
Compensation:
Hourly Pay: $21 - $27 (D.O.E.) + Full Benefits
Shifts: First and Mid-Shift Available
