Location: Joshua Tree, CA
Schedule: Full-Time | Days (Monday – Friday, 8:30 AM – 5:00 PM)
Compensation: $83,200 – $131,809 annually
Incentives: Up to $20,000 sign-on bonus (based on experience)
Relocation Assistance: Possible for ideal candidate

About the Role
We are seeking a dedicated Physical Therapist (PT) to join our Home Health team in Joshua Tree, CA. In this role, you will provide physical therapy services to patients in their homes, focusing on rehabilitation, functional independence, and quality of life. This is an excellent opportunity for a motivated therapist who thrives in an independent, patient-centered environment while enjoying the flexibility of home health care.
Primary Duties
- Evaluate patients and develop individualized treatment plans with specific goals and activities

Provide direct physical therapy per plan of care and adjust as needed

Instruct patients and caregivers in therapeutic procedures to be continued at home

Document evaluations, treatment plans, progress notes, and reports in compliance with standards

Collaborate with the interdisciplinary team and attend patient care conferences

Educate patients and families on progress, expectations, and home exercise programs

Participate in quality improvement programs, staff inservices, and professional development

Demonstrate cultural sensitivity, especially when caring for geriatric and end-of-life patients
Qualifications
- Education: Bachelor’s, Master’s, or Doctorate in Physical Therapy (from accredited program)

Experience: Minimum 2 years of physical therapy experience (home health experience preferred)

Licensure & Certifications:

Current California Physical Therapy License
-
CPR certification
-
Valid California driver’s license and proof of automobile insurance

Skills & Abilities:

Strong documentation, organization, and communication skills

Ability to work independently while managing a home health caseload

Knowledge of infection control, patient safety, and proper body mechanics

Physical Demands

Prolonged standing and walking

Lifting, positioning, and transferring patients

Handling therapy equipment and supplies
