Director of Case Management
We are seeking a dynamic Director of Case Management to lead our facility's case management department. The Director will have overall responsibility for managing and coordinating all department activities.
Responsibilities:
- Provide leadership and oversight for all aspects of daily case management operations. Ensure adherence to care coordination policies and timely discharge planning.
- Direct and evaluate departmental operations including case management model design, staffing, use of technologies, training, and competency development.
- Allocate resources to effectively staff the department and meet productivity and quality goals. Maintain optimal staffing mix and levels.
- Oversee core case management functions and practices. Ensure compliance with standards of practice and regulations.
- Partner with facility leadership and across departments to achieve goals for clinical outcomes, resource utilization, and continuity of care.
- Assess and improve department performance through data analysis, audits, and implementing process improvements.
- Establish effective working relationships with key stakeholders including medical staff, nursing, and ancillary services.
Requirements:
- Current RN license and Bachelor's degree in Nursing (required)
- Master's degree in Nursing, Health Administration or Business Administration (preferred)
- 3+ years of acute care case management experience
- 2+ years in a case management leadership role
- Experience as a Director in an acute care setting
- Ability to lead, motivate and develop a large team
- Strong communication, collaboration, and analytical skills
