A Director of Case Management is sought for a 165-bed acute care hospital in North Richland Hills, TX — a Primary Stroke Center and accredited Cycle 5 Chest Pain Center with a cardiac rehabilitation program, comprehensive cardiovascular services, spine surgery, joint replacement, minimally invasive robotic surgery, hyperbaric oxygen therapy, and a geriatric behavioral unit.

The role. The Director holds full operational accountability for hospital case management, leading a team of 10 FTEs and 7 PRN staff across a facility that treats nearly 33,000 patients annually. Reporting closely to the CFO, CMO, and Medical Director, the Director oversees the case management model end-to-end — including staffing mix, onboarding, competency development, and technology utilization — to meet performance and quality objectives. A core focus is ensuring the integrity of care coordination and discharge planning processes: timeliness, documentation quality, and continuity of care across interdisciplinary teams and external agencies. The Director partners with the CFO and Division Case Management Leader to drive financial and operational goals, and establishes collaborative relationships with the CMO, CNO, ancillary service leaders, and compliance and legal stakeholders. Ongoing performance improvement is central to the role, requiring the Director to analyze operational data, identify gaps, and implement sustainable solutions.

What we're looking for.
  • Bachelor's degree required
  • Master's degree in Nursing, Health Administration, or Business Administration preferred
  • Active Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
  • 3+ years of acute care hospital case management experience required
  • 2+ years of case management leadership experience required

Schedule. Full-time, day shift, Monday through Friday only, with shifts ranging 7–10 hours; some on-call rotation or extended days may be required.

Compensation. $102,107 - $135,000 per year plus $18 sign-on bonus
