A Case Manager RN opportunity is available at a 267-bed hospital in Fort Walton Beach, FL, offering an Advanced Primary Stroke Center, a Cancer Care Center, open-heart surgery services, and emergency care across a main ER, a pediatric ER, and two freestanding ERs.

The role. Reporting to the Rehab Program Director and Facility Case Management Director, this position guides patients and families through every step of their care journey — from program orientation and explanation of Medicare and insurance benefits to discharge criteria and patient rights. The Case Manager conducts psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the interdisciplinary care team. Working closely with physicians, nurses, and department leaders, this role develops and implements individualized treatment plans that reflect each patient's strengths, needs, and personal recovery goals. Coordinating education for patients, families, and caregivers is a key component, encouraging active participation in treatment planning, goal discussions, and family conferences to support safe, seamless transitions across the continuum of care.

What we're looking for.
  • Associate Degree or Certificate
  • Registered Nurse (RN) license
  • 2 years of recent (within the last 3 years) acute care experience; ICU, CCU, Med-Surg, or LTAC Case Management experience preferred

Schedule. Days with rotating weekends, 8:00 AM to 4:30 PM.
